What to put on a embroidery order form

  • One-off setup charges as their own lines

    Digitising and screen setup are real costs that don't repeat on reorders. Give each its own priced line, capped low, so the first order carries them and the reorder doesn't — and the total is right either way.

  • Garment options that include or exclude the blank

    Some customers buy your blanks; some bring their own. Separate lines for "supplied on our blank" and "decoration only" price both paths without a conversation.

  • Method and placement as required choices

    Embroidery and screen printing are different jobs on different machines. Capturing method and placement up front stops a quote built for the wrong process.

  • A design reference plus a proof step

    Custom art is where reprints hide. A named design and a stated "we'll proof before we run" step turns a vague brief into an approval you both signed off.

The garment (yours or customer-supplied), quantity and sizes, the decoration method, placement, any one-off setup charges like digitising or screen setup as their own lines, a design reference, and a proof-approval step.

+How do I handle digitising and screen setup charges?

Put each as its own priced product line the customer adds — never as a fee inside a dropdown option. Only priced product lines are added to the total, and a separate line makes it obvious the charge is one-off and reused free on reorders.

+Can one form handle both embroidery and printing?

Yes — the method field covers both, and the setup lines (digitising for embroidery, screen setup per colour for printing) sit side by side. For a job that needs both on one garment, capture it in the method choice and spell it out in the art notes.
