Sizes priced by servings, not just inches
Customers think in "how many people", not in diameters. Putting "serves 14–16" in the product name cuts the back-and-forth and stops undersized orders.
Flavour and filling as separate required choices
Combining them into one dropdown creates a combinatorial mess. Two required dropdowns give you a clean ticket for the kitchen and no blanks.
The inscription as free text, quoted back
Piping is the one thing you cannot undo. Capture the exact string the customer typed and read it back in the confirmation — spelling disputes end there.
A required pickup date
It's your capacity control. With a date on every order, your dashboard sorts into a production schedule instead of a pile of messages.
Allergies as one open line, not a tick-box with a "none" option
A "no allergy concerns" box can be ticked next to "nut-free needed", and the kitchen gets a ticket that contradicts itself. One line of free text can't do that — and read it back in the confirmation so the customer can correct you. If an allergy option changes the price, like a gluten-free sponge, put it in the order as a priced product line: only priced lines reach the total.

Size in servings, flavour, filling, inscription text, pickup date and time, allergy notes, a design/inspiration box, and the customer's phone number. Price and deposit terms should be visible on the form itself, not sent afterwards.
Most small bakeshops ask two weeks for custom work and 72 hours for standard designs. Put those windows in the form description so the expectation is set before someone picks a date.
The form captures the order and total; you send the deposit request yourself — a payment link in your confirmation email works well. The Excel version calculates a 50% deposit and remaining balance for you.
Not on the form today. The design notes box is the place to describe it, and most bakers ask customers to reply to the confirmation email with photos. File uploads are on the roadmap.
