One line per size, not a size dropdown
A dropdown gives you one size per order; separate lines let a customer order two mediums and a large in a single submission — and hand you a cut count you can send straight to the supplier.
Extended sizes priced on their own line
2XL and up cost more to buy blank. Charging them at cost on a separate line keeps your margin intact and avoids the awkward top-up conversation later.
Embroidery as a priced product line, not a free promise
Personalisation is labour. Anything with a price must be a product line so the total computes — add a $6 embroidery line the customer selects per garment, capped to the run. "+$6 for a name" typed into a dropdown option is only text, and the order arrives short.
Colourway as a single required choice
A blank colour field is the single most common cause of a remake. Make it required, and use the notes box for the rare order that wants a different colour per garment.

The Word version is a fully editable order sheet — swap in your garments, your prices and your colourways, add your logo, and print it for the counter or email it to a group organiser.
The spreadsheet does the arithmetic and the tally at once. Quantity × price is a live formula, and the log sheet totals the size run so you order exactly the blanks you need.
The PDF is the version you send when you don't want anyone editing the prices. Fixed US Letter layout, generous writing lines, and it reads the same on a phone as on paper.
