Teams, schools & workplaces
Uniform orders repeat every season and break the same way every time: a size guessed, a squad number duplicated, a name spelled off a text message. The reorder is never at the first-run price.
This template collects kit the way a team or a workplace actually issues it — a line per size, number and name printing as their own priced lines, and a wearer field so every set is sized to a person, not a guess.

Field by field
One line per size, not a size dropdown
A team spans youth and adult sizes across one order. Separate lines give you a clean size run for the supplier instead of a spreadsheet you have to re-sort by hand.
Number and name printing as priced lines
Personalisation is a real cost and a real decision. Give each its own product line the customer adds, then capture the exact number and text in a field — a price in a dropdown option is only text and never reaches the total.
A required wearer name
Kit is issued per person. A required name field ties each set to a player, so handout day is a name check, not a guessing game.
A number-clash confirmation in the description
Squad numbers collide. Saying "first come, first served, we'll confirm clashes" up front turns an argument into an expected step.

Edit the Word version into your team's or workplace's kit list — your garments, sizes and prices — and print an order slip for every player or employee.
The spreadsheet totals each order and, on the log sheet, tallies the whole squad — sizes, numbers and names in one place ready to send to the supplier.
Send the PDF as the season order sheet — fixed prices, one page, and it reads the same on a parent's phone as on the noticeboard.
The take-home slip: print one per player or employee, with boxes for size, number and name, and a signature line for the deposit.
Same fields, same prices — but the totals calculate themselves, the orders land in one dashboard, and nothing gets lost in an inbox. Share one link anywhere: bio, group chat, QR code on a flyer.

Each garment and size as its own priced line, number and name printing as separate priced lines, a required wearer name, the size and print details in a field, the closing date, and delivery timing.
Collect the requested number in a text field and confirm allocations yourself — state in the description that numbers are first come, first served. The online dashboard lets you scan every requested number in one view.
Add each as its own product line the wearer selects, not as a price inside a dropdown option. Only priced product lines are added to the total, so a print fee in an option label looks charged but isn't.
Yes. Duplicate the file, or with the online version edit the sizes and dates and reuse the same link — last season's orders stay in your dashboard for reference.
