One line per thing you sell, with a price
The moment prices live on the form, totals stop being arithmetic you do by hand at 11pm — and stop being wrong. The rule holds for extras too: delivery, gift wrap, a rush fee. Anything with a price must be a product line so the total computes — cap it at one per order if that is what it is. A price written into a dropdown option is only text and is never added up.
A required date-needed field
Almost every order dispute is really a scheduling dispute. One required date field prevents most of them.

Never assume. A three-option dropdown decides how the order is fulfilled before you've committed to anything.
A phone number, not just an email
When something changes on the day, email is useless. Turn on phone collection for anything time-sensitive.
One open notes box — and only one
A single "special instructions" box catches the exceptions. More than one free-text box and people stop filling any of them in.

An order form is a short document that records what a customer wants to buy: the items, quantities, unit prices, line totals and order total, plus who they are, when they need it, and how it's being delivered. It's the written record both sides rely on.
Business name, customer name and contact details, an itemised list with quantity, unit price and line total, an order total, a date needed, a delivery or pickup choice, and one box for special instructions.
Word if you want to rewrite the wording and branding, Excel if you want the totals calculated for you, PDF if you're sending it out and don't want anything edited. The printable PDF is the one to keep on a clipboard.
Download the .docx or .xlsx and upload it to your Google Drive — Drive converts it into Docs or Sheets automatically and the spreadsheet formulas survive the trip. We don't host "make a copy" Google links, because those have to live in someone's Google account.
