Field by field

What to put on a t-shirt order form

  • One line per size, not a size dropdown

    A dropdown gives you one size per order; separate lines let a parent order an M, an L and a youth tee in a single submission — and give you a print count you can send straight to the shop.

  • Extended sizes priced on their own line

    2XL and up cost more to print. Charging them at cost on a separate line stops the awkward conversation later and keeps your margin intact.

  • Ink or colourway as a single choice

    If you're offering more than one colourway, make it required — a blank colour field is the single most common cause of a reprint.

  • Pickup or ship — with postage as its own priced line

    Half of every t-shirt order goes to people who won't be at the pickup. Ask once, up front, and give postage a product line of its own. Anything with a price has to be a product line so the total computes: a figure typed into a dropdown option is only text, and it never reaches the total.

+Is this t-shirt order form template free?

+How do I handle sizes on a t-shirt order form?

List each size as its own line with its own price rather than using a size dropdown. One person often orders several sizes, and a per-size line gives you a print run count you can hand to the printer without re-counting.

+Should I charge more for 2XL and 3XL?

Most print shops charge $2–$4 extra above XL. Put the surcharge in the price of that line rather than adding it at the end — people accept it when they see it next to the size, and it keeps your totals honest.

+Can I collect payment through the form?

Not yet. Today the form collects the order and the total; you settle payment however you already do — cash at pickup, a payment link in your confirmation email, or an invoice. In-form payments are on the roadmap.

+Can I use this template in Google Docs or Google Sheets?

Yes, with one extra step: download the .docx or .xlsx, upload it to your Google Drive, and open it — Drive converts it to Docs or Sheets automatically. We don't host copy-this-file Google links, so this is the honest route.
