Florists & flower studios
A flower order lives or dies on two details: the date it has to land, and the exact words on the card. Miss the date and the apology is expensive; misspell the card and it's the one thing the recipient photographs.
This template captures a florist's order the way you actually fill it — arrangements priced by size, a required delivery date, delivery charged on its own line, and the card message as free text you read back word for word.

Field by field
Arrangements priced by size, not by stem
Customers choose in "small, medium, luxury", not stem counts. Naming the size and setting the price there cuts the back-and-forth and stops an under-scaled order arriving.
A required delivery date
It's the whole job. A required date field sorts your bench into a delivery run and refuses the order with no date attached — the one that always turns into a same-day scramble.
The card message as free text, read back
The card is the part the recipient keeps. Capture the exact words the customer typed and confirm them in your reply — spelling and wording disputes end there.
Delivery as its own priced line
Absorbing delivery quietly eats a florist's margin. Anything with a price must be a product line so the total computes — a flat-rate line the customer adds, capped at one. A fee written into a dropdown option is only text and never gets charged.

Edit the Word file into your own studio order sheet — your arrangements, your palettes, your delivery terms — and print a pad for the counter or email it to a corporate account.
The spreadsheet totals each order and, on the log sheet, becomes your delivery diary — one row per order, sortable by delivery date so you can plan the run.
Send the PDF when a customer asks what you need. A fixed, one-page brief covering size, palette, date, card and address — everything you'd otherwise extract over a string of DMs.
The counter form: one page, big writing boxes, and a card-message block wide enough to copy legibly. The date and address sit near the top where they're hard to miss.
Same fields, same prices — but the totals calculate themselves, the orders land in one dashboard, and nothing gets lost in an inbox. Share one link anywhere: bio, group chat, QR code on a flyer.

The arrangement and size with its price, a colour palette, a required delivery or pickup date, the card message as free text, the recipient's name, address and a contact number, any flowers to avoid, and the customer's own phone number.
48 hours is a common minimum for a made-to-order arrangement, longer for peak weeks like Valentine's and Mother's Day. Put the lead time in the form description so the expectation is set before someone picks a date.
Add delivery as its own product line with your flat rate, capped at one per order — not as a fee written into a dropdown option. Only priced product lines are added to the total, so a fee in an option label looks charged but isn't.
Not as a file on the form yet — the notes box is where they describe it, and most florists ask customers to reply to the confirmation with a photo. File uploads are on the roadmap.
