Schools, offices & lunch programs
A lunch program is the same order, fifty-two weeks a year: who's eating, on which days, and what they can't have. Run it on paper and every Monday is a fresh pile of slips to count before the kitchen can start.
This template collects a week of lunches the way a school or office program actually runs it — meals priced per day, a required diner name, and dietary notes captured once so the kitchen never cooks blind. It's also the order form with the best repeat rhythm: the same link, every week.

Field by field
Meals priced per day, with a bundle option
Families pick some days and skip others, so each day is its own line. A discounted full-week bundle captures the regulars — just tell people not to tick both.
A required diner name and room
It's how a tray gets to the right desk. Name labels the meal; room routes the delivery. Both required means no unlabelled lunch in the cart.
A per-day cap of five
Nobody eats six Mondays. Capping each day at the number of school days stops fat-finger orders and keeps the kitchen count sane.
Dietary notes captured once, in writing
Allergies in a lunch program are non-negotiable. One free-text line, filled in with the order and read back, beats a note remembered from September.

Edit the Word version into next week's menu — it changes as the menu changes, and prints as a take-home slip or a staff-room sign-up sheet.
The spreadsheet totals each order and the log sheet counts meals per day — the number the kitchen needs by Friday afternoon to shop and prep for Monday.
Send the PDF as the weekly menu — a fixed, one-page sheet of days, meals and prices parents or staff can't accidentally edit.
The take-home slip: print a stack, tick the days, write the name and room, and send it back with payment.
Same fields, same prices — but the totals calculate themselves, the orders land in one dashboard, and nothing gets lost in an inbox. Share one link anywhere: bio, group chat, QR code on a flyer.

Each day's meal as its own priced line, an optional full-week bundle, a milk or add-on line, a required diner name and room, an allergy and dietary notes line, and the ordering deadline for the week.
That's the online version's strength: edit the week's menu and reuse the same link, so families order from a bookmark and orders land already labelled by name and room. The kitchen count updates as each order comes in.
Collect them as a free-text line on every order and read them back in your confirmation. For the online version, sort or export the dashboard so the kitchen has the full allergy list before prep.
Yes — the full-week bundle is a single priced line at a small discount. The form captures the order and total; you collect payment your usual way, and the Excel log totals what's owed.
