Copy the supplier's sheet exactly — the totals the form calculates are the ones you'll reconcile against when the invoice lands.
Sold by (dropdown of your roster)
The single most useful field on the form. It credits the seller and tells you which family to hand the boxes to.
Buyer name and phone (required)
Distribution day runs on names, and a phone number means one text covers a change of pickup time.
Pickup or delivery (single choice)
Aunts across town versus neighbors down the street — knowing the split before delivery week saves a lot of driving.
